All major credit cards can be accepted as a form of payment. Included in the intake forms will be a credit card authorization form where you can allow me to safely store a card – charging the card only after each session. It is required for all clients to have a card on file, to comply with late-cancellation/no-show policies.
Cancellation Policy: If you are unable to attend a session please make sure you cancel at least 24 hours beforehand. Otherwise you will be charged $140 for a late cancellation fee with the card provided on file.
Please note: I do NOT accept MEDICARE or MEDICAID, under any insurance plan. I am also not allowed to take any cash rate from a Medicaid/Medicare member.
